Successor: The maxillary second premolar succeeds the deciduous second molar and sits between the 1st premolar and the 1st molar in the arch.

Quick ID: No marginal groove + 1 canal + oval occlusal + equal cusps = 2nd premolar.
👁️ Buccal & Lingual Aspects
- Buccal cusp tip is less displaced mesially than the 1st premolar's
- Mesial and distal cusp slopes are more equal than in the 1st premolar
- From the lingual aspect, the lingual cusp is closer in height to the buccal cusp (more equal cusps overall)
⬆️ Occlusal Aspect
- Oval to circular outline — no mesial concavity, so no hexagon shape
- Shorter central developmental groove than the 1st premolar's
- More numerous supplemental grooves, giving a "wrinkled" occlusal appearance
- Mesial and distal fossae are more equal in size (unlike the 1st premolar where mesial dominates)
🦴 Root & Pulp Cavity
- Usually single root, single canal — much less commonly bifurcated than the 1st premolar
- Root is more symmetrical, with less of a mesial depression
- 2 pulp horns, but only 1 canal
- Cervical cross-section: oval, wider buccolingually, but more symmetrical than the 1st premolar's kidney shape

Shorter Cusp, Convex Mesial Outline
Unlike the 1st premolar, the mesial outline here is convex — not concave.
Lingual
Lingual Cusp Closer in Height to Buccal
The two cusps are more equal in height than in the 1st premolar.
🎯 Mesial Aspect — Key Diagnostic Difference
Mesial
NO Mesial Marginal Ridge Groove
This is the single most important diagnostic difference from the 1st premolar — the groove is simply absent here.

No mesial concavity means no hexagon shape — just an oval outline with extra supplemental grooves.

A closer look shows fossae that are more equal in size than the 1st premolar's larger mesial fossa.

⚠️ Exam Trap: Remember the quick ID formula: no marginal groove + 1 canal + oval occlusal + equal cusps = maxillary 2nd premolar. The reverse combination identifies the 1st premolar.
